In the Dominican Republic, during one of the bloodiest dictatorships in the history of Latin America, a group of mental patients escape from the state-run mental hospital. In order to calm the negative press generated by the institution's lack of control, Dr. Antonio Zaglul is appointed its new director. Once there, Zaglul will find it difficult to tell if the madness lives inside or outside the hospital walls.
